Bất thường di truyền trong vô sinh nam

TÓM TẮT:

Mục tiêu: Cập nhật những kiến thức cơ bản và khá đầy đủ về những bất thường di truyền trong vô sinh nam, chủ yếu là ở mức độ tế bào học.

Phương pháp: Viết lại một cách có hệ thống dựa trên tham khảo y văn trên thế giới về lãnh vực bất thường di truyền trong vô sinh nam (có bổ sung một số dữ liệu trên người Việt Nam).

Kết quả: Các biểu hiện lâm sàng của vô sinh nam từ suy hạ đồi tuyến yên đến sự tổn hại của quá trình sinh tinh (thiểu tinh nặng, không tinh trùng không do bế tắc) đến không tinh trùng do bế tắc đều có liên quan đến các khiếm khuyết về di  truyền như: sai lệch nhiễm sắc thể về số lượng và cấu trúc và đột biến gen xơ hóa nang. Các sai lệch nhiễm sắc thể về số lượng chủ yếu đề cập đến các bất thường số lượng nhiễm sắc thể giới tính như: 47,XXY; hội chứng nam 46,XX; 47,XYY làm tổn hại quá trình sinh tinh (thiểu tinh nặng và không tinh trùng). Các bất thường cấu trúc nhiễm sắc thể giới tính như đảo đoạn, chuyển đoạn có thể làm suy kém sự sinh tinh đưa đến thiểu tinh. Chúng ta đặc biệt chú ý đến mất đoạn nhỏ trên  nhiễm sắc thể Y bao gồm các vùng AZFa, AZFb, AZFc gây khiếm khuyết quá trình sinh tinh đưa đến thiểu tinh nặng và không tinh trùng. Các sai lệch về cấu trúc nhiễm sắc thể thường dẫn đến những bất thường trong vô sinh nam dưới 2 hình  thức biểu hiện lâm sàng là thiểu tinh hoặc sảy thai liên tiếp.

Kết luận: Ngày nay, nhờ các kỹ thuật hỗ trợ sinh sản, đặc biệt là kỹ thuật ICSI đã tạo cơ hội cho những trường hợp vô sinh nam trầm trọng được làm cha. Tuy nhiên, những người vô sinh nam này có nguy cơ truyền những bất thường di truyền cho  con cái. Do đó họ nên được tư vấn về những nguy cơ này trước khi được điều trị bằng kỹ thuật hỗ trợ sinh sản để tránh cho thế hệ sau những bất thường di truyền, xét trên từng cá thể và của cả cộng đồng.

ABSTRACT:

Purpose: We update the basic and rather sufficient knowledge of genetic abnormalities of male infertility, mainly in cytogenetics aspect.

Materials and methods: To rewrite systematically on genetic abnormalities of male infertility base on referring to the world wide literature on this aspect (supplement some facts and figures on Vietnamese infertile men).

Results: The variety of clinical presentation infertile man ranging from  hypogonadotropic hypogonadism to spermatogenic failure (severe oligospermia, non-obstructive azoospermia) to obstructive azoospermia are associated with  genetic defects including numerical and structural chromosomal aberrations and cystic fibrosis transmembrane regulator mutational gene. Numerical chromosomal aberration mainly involved sex chromosome such as: 47,XXY; 46,XX male; 47,XYY cause spermatogenic failure (azoospermia and severe oligospermia). Structural sex chromosomal aberrations such as inversions, translocations may impair spermatogenesis, resulting in oligoasthenotetratozoospermia. We especially pay attention to Y chomosome microdeletions including AZFa, AZFb, AZFc related to spermatogenic defects with extreme oligospermia and azoospermia. Structural  abnormalities of the autosomes may cause disturbances of male infertilities, resulting in 2 different clinical manifestations: oligospermia or successive  abortion.

Lastly, cystic fibrosis transmembrane regulator mutational gene leads to congenital bilateral absence of vas deferens, resulting in obstructive azoospermia.

Conclusion:  Nowsaday,thanks to assisted reproductive techniques (ARTs), especially ICSI  create opportunity for severe infertile men to father children. However, they  have the risks of passing genetic abnormalities to the offspring. Therefore,  they should be counseled about these risks before applicated of ARTs in order to avoid creating future generations of genetically abnormal community and each individual as well.
