Đặc điểm viêm phổi ở trẻ sơ sinh tại bệnh viện nhi đồng 2 từ 03/2007 đến 10/2007

TÓM TẮT

Viêm phổi vẫn còn là nguyên nhân quan trọng gây bệnh tật và tử vong ở sơ sinh non tháng và đủ tháng. Trẻ sơ sinh dễ viêm phổi có thể liên quan đến sự chưa trưởng thành của hệ thống thanh lọc nhầy-lông, kích thước đường hô hấp nhỏ và sức đề kháng kém.

Mục tiêu: Mô tả đặc điểm dịch tễ học, lâm sàng, cận lâm sàng và điều trị viêm phổi ở trẻ sơ sinh nhập bệnh viện Nhi Đồng 2.

Phương pháp: Tiền cứu, mô tả hàng loạt ca.

Kết quả: 331 bệnh nhi vào lô nghiên cứu trong đó có 74 trẻ sanh non. Có 73 ca (22,1%) viêm phổi sớm, 258 ca (77,9%) viêm phổi trễ và 49 ca (14,8%) viêm phổi bệnh viện. Có 63 ca (19%) có yếu tố nguy cơ từ mẹ và có liên quan với viêm phổi sớm. Triệu chứng hô hấp thường gặp nhất là ran phổi, thở co lõm ngực và ho (79,3%; 78% và 69,8%). Triệu chứng không đặc hiệu thường gặp nhất là bỏ bú hoặc bú kém và ọc sữa (68,5% và 51,5%). XQ phổi có hình ảnh thâm nhiễm phế nang 79,2%. Bạch cầu, CRP trong giới hạn bình thường là 85,8% và 77%. Cấy máu dương tính 20 ca (8,7%). Trẻ cần hỗ trợ hô hấp 139 ca (42%) trong đó 41% thở máy; 37,4% thở oxy và 21,6% thở NCPAP. Tỉ lệ tử vong do viêm phổi trong nghiên cứu là 7,3% (24 ca) và có mối liên quan với nhiễm trùng huyết, tim bẩm sinh và sanh  ngạt.

Kết luận: Đa số trẻ có yếu tố nguy cơ từ mẹ có liên quan với viêm phổi sớm nên cần theo dõi những trẻ này để phát hiện sớm VP. Tử vong trong viêm phổi sơ sinh thường kèm với các bệnh lý như nhiễm trùng huyết, tim bẩm sinh, sanh ngạt do đó cần theo dõi điều trị tích cực những trẻ này.

Backgrounds:
Pneumonia remains a significant cause of morbidity and mortality for preterm and term neonates. The increased susceptibility of  neonates for pneumonia may be related to immaturity of mucociliary clearance, small size of the conducting airways and lowered host defenses.

Objective: To describe characteristics of the epidermiology, clinical, paraclinical and treatment of neonatal pneumonia at Children Hospital N0 2.

Methods: Case series.

Results: A total of 331 patients were enrolled in which there were 74 (22,4%) preterm neonates. There were 73 (22,1%) cases of early onset pneumonia, 258 (77,9%) cases of late onset pneumonia and 49 (14,8%) cases of nosocomial  pneumonia. Mothers risk factors were present in 63 (19%) cases and related to  early onset pneumonia. Rales, chest indrawing and cough were the most common respiratory symptoms: 79,3%; 78% and 69,8% respectively. Poor feeding and vomiting were the most common nonspecific signs: 68,5% and 51,5% respectively.  Chest X-ray showed alveolar infiltrates in 79,2%. White blood cells and C reactive protein were normal in most of cases (85,5% and 77%). Blood cultures were positive in 20 cases (8,7%). One hundred and thirty nine cases (42%) were needed to supply oxygen: mechanical ventilations 41%; nasal canula 37,4% and  nasal continuous positive airway pressure 21,6%. The mortality of neonatal  pneumonia was 7,3% and related to sepsis, congenital heart diseases and birth  asphyxia.

Conclusions: We should close monitor neonates who have mothers  risk factors to diagnose pneumonia early because the risk factors related to early onset pneumonia. The high mortality of neonatal pneomonia usually associated with sepsis, congenital heart diseases and birth asphyxia. Therefore, these neonates should be close monitored and treated intensively.
